There are two fields which must be initialed and signed. For all warranty units please initial the box about halfway down the page and also sign by the X_______ towards the bottom of the invoice. If the unit is out of warranty, please sign just by the X_______. For out of warranty units we will contact you to make arrangements for payment of the estimate fee. Please pack your unit well and insure it for the value you desire. Please list the amount you would like us to insure it for when we ship it back to you. Put this amount next to your signature X_______. We will contact you once the diagnosis has been made.
